What Can You Expect from NPTC Homecare?
Under the direction of a supervisor, the Homecare Program services are provided by a registered nurse, licensed practical nurse, and a certified health care aide.
Once a referral is received the homecare nurse will contact you to set up in an initial visit.
During a initial visit homecare staff will meet you at an arranged location. The staff will complete a physical assessment (i.e; listening to heart and lungs, etc.), vital signs and also numerous health related questions. In addition you (or legal guardian) will be asked to sign release forms.
The duration and frequency of future visits will be decided by the Homecare team in cooperation with the client and family members.
NPTC Homecare promotes client independence and encourages family and friends to participate in client care.
Confidentiality and Respect are very important to the Homecare staff. You can be assured that all measures will be taken to uphold these principles.
Anyone can make a referral to homecare: Family & Friends, Dr, Hospital or a Community Member.
